Optional Midterm Feedback is conducted differently from end of course evaluations. In the "Quizzes" tab of the Canvas course, instructors will find a quiz entitled, "Midterm Student Feedback Survey". This ungraded and anonymous survey consists of two basic qualitative questions. It allows students to provide feedback directly to the instructor, and the instructor can begin generating the feedback reports the moment a student has responded. The instructor can add custom questions to this form, set dates, and publish at their preference. For assistance with setting dates, instructors may reach out to Samberg. Note: this midterm feedback is required for new courses and new instructors in the first course iteration.
A few important things to know:
- You are not asked to provide any time in class for students to complete this form. Students can complete the form on their own time.
- Students can access the form simply by clicking the "Quizzes" tab in your Canvas course.
- Feedback is truly anonymous.
- You are able to access the feedback throughout the entirety of its collection by: 1) clicking on the survey title, 2) clicking Survey Statistics in the upper-right, and 3) clicking the Student Analysis button to generate an Excel output.
- The form consists of the two questions listed below, but you can customize and add to these if you’d like. Please let us know if you’d like our help to do so. Students are also reminded to make their feedback behavior-focused and solution-oriented.

- Please list two to three things about the course and instructor/s that you think could be improved (e.g. content, class facilitation, delivery, etc.).
